Start off with the name of the char, ip, reason, admin that banned and maybe some pictures if you have

Example:

Name: Jokke
Ip: 111.111.11.11
Banner: IAMtheone
Reason: i said fu** to the admin and dm alot.
Picture: Howto send app! Post link here

And an admin will be on soon to validate the app.
